It is not always about fear. It is sometimes about dominance. My whole male, 4yo, is not the sort to just put up with poking and sticking. Even with me there, he will assert himself. He is completely safe with dogs, cats and people, but not with someone trying to assume dominant positions. I do have to muzzle him when the vet will have to be invasive (eg examine an ear canal).
